    Abstract

    Review examining CoQ10's role in cardiovascular health, including effects on heart failure and vascular function.

    Methodology

    Review of the biochemistry of coenzyme Q10 and of clinical trial evidence in heart failure, hypertension, endothelial function and statin-associated muscle symptoms.

    Key Findings

    Myocardial CoQ10 levels are reduced in advanced heart failure and correlate with severity. Supplementation improved functional class and, in the Q-SYMBIO trial, major adverse cardiovascular events. Effects on blood pressure and endothelial function are modest, and evidence for statin-associated muscle symptoms is inconsistent.

    Conclusions

    CoQ10 has a plausible mechanistic and clinical rationale as an adjunct in chronic heart failure; evidence for other cardiovascular indications remains weaker and should not displace standard therapy.
