TL;DR
Genuinely high prolactin is a medical finding that needs a cause identified — often a medication or a pituitary adenoma — not a supplement. Vitex lowers prolactin mildly and has a legitimate role in cyclical breast pain and luteal phase issues.
Why It Matters
Prolactin is secreted by the anterior pituitary under tonic dopaminergic inhibition. Elevated levels suppress GnRH, causing irregular or absent periods, low libido, erectile dysfunction, galactorrhoea and infertility. The common causes are antipsychotics and other dopamine-blocking drugs, hypothyroidism, prolactinoma, chest wall stimulation, and stress or exercise before the blood draw. Any confirmed elevation needs thyroid testing and usually pituitary imaging — self-treating with herbs can delay diagnosis of an adenoma.

1) Confirm any elevated result with a rested morning repeat — stress, recent exercise, nipple stimulation and even the needle can raise prolactin transiently. 2) Rule out pregnancy, then check thyroid function; hypothyroidism is a common and easily fixed cause. 3) Review medications — antipsychotics, metoclopramide, some antidepressants and opioids are frequent culprits, and changing them is the actual treatment. 4) Persistently high levels warrant endocrinology referral and imaging; prolactinomas respond very well to dopamine agonists. 5) Support normal regulation with sleep, stress reduction and avoiding chronic overtraining. 6) Vitex 20-40mg/day is reasonable for mild luteal-phase symptoms and cyclical breast pain, but not as a substitute for investigating a high result.
