Premature graying is mostly written in your genes — look at when your parents grayed. The testable, treatable causes are B12 deficiency, thyroid disease, and smoking, worth checking if graying is rapid or very early. No supplement reverses established gray hair; biotin marketed for it has no supporting evidence. Sudden patchy graying with autoimmune symptoms deserves a medical look.
Overview
Graying before the expected age — before the mid-20s in most ethnic groups, mid-30s in others — driven overwhelmingly by genetics. Reversible causes exist (B12 deficiency, thyroid disease, severe stress, smoking) but account for a minority of cases, and no supplement repigments hair once the pigment cells are gone.
Root Causes
Evidence synthesis: Genetics dominates — genome studies link premature graying to variants (IRF4) and family history is the strongest predictor. B12 deficiency is an established reversible cause, with case series documenting repigmentation after repletion; thyroid dysfunction and vitiligo-related autoimmunity are also documented causes. Smoking is associated with 2.5x increased risk of premature graying in observational data. Oxidative stress is the proposed common mechanism, but antioxidant supplements have never demonstrated repigmentation in trials. Biotin, heavily marketed for hair, has no trial evidence for graying — it addresses deficiency-related hair loss only. Once melanocyte stem cells in a follicle are depleted, the change is currently irreversible.
