Overview
Hearing changes divide cleanly into conductive problems, where sound cannot reach the inner ear, and sensorineural problems, where the cochlea or auditory nerve is damaged. Wax impaction, middle ear fluid, and eardrum perforation are conductive and usually reversible. Age-related loss (presbycusis) and noise-induced loss are sensorineural and permanent, because the hair cells of the cochlea do not regenerate in humans. The pattern of difficulty is informative. Struggling to follow conversation in a noisy restaurant while hearing fine one-to-one is the classic early presbycusis picture — high-frequency loss strips out the consonants that carry meaning. Muffled hearing on one side after a cold may be middle ear fluid. Sudden sensorineural hearing loss, defined as 30 dB or more across three frequencies within three days, is a medical emergency where high-dose oral or intratympanic steroids started early meaningfully improve recovery odds. Untreated hearing loss is one of the largest modifiable risk factors for dementia in the Lancet Commission analysis. Hearing aids are the intervention with real evidence; no supplement restores hearing.

Root Causes

Conductive loss reflects a mechanical barrier — wax, fluid, or ossicular disease. Distinguishing the two determines whether the problem is fixable or must be managed with amplification.
