Overview
Chronic kidney disease is defined by an estimated glomerular filtration rate below 60 mL/min/1.73m2, or evidence of kidney damage such as albuminuria, persisting for at least three months. It is staged G1 to G5 by filtration rate and A1 to A3 by albuminuria, and both axes matter — a person with preserved filtration but heavy albuminuria can be at higher risk than someone with mildly reduced filtration and no protein loss. Most cases arise from diabetes and hypertension, and most progression is preventable rather than inevitable. The interventions that alter the trajectory are well defined: blood pressure control, renin-angiotensin system blockade where albuminuria is present, and SGLT2 inhibitors, which reduced kidney failure and cardiovascular death across DAPA-CKD and EMPA-KIDNEY regardless of diabetes status. That last finding reshaped the field. Nutritional management becomes progressively more restrictive as filtration falls. Potassium and phosphate accumulate, protein intake needs moderation in advanced stages, and sodium restriction supports blood pressure control. The supplement implications are unusually stringent. Potassium and magnesium supplements can cause dangerous accumulation. Vitamin C above modest doses risks oxalate deposition. Fat-soluble vitamin A accumulates. Many herbal products, including aristolochia-containing preparations, are directly nephrotoxic, and NSAIDs accelerate decline. Vitamin D deficiency is common and repletion is standard, but activated forms require specialist supervision.
