    Komesaroff and colleagues randomized 23 women with menopausal symptoms to topical wild yam cream or placebo for three months each in a double-blind crossover design, measuring symptom diaries alongside serum and salivary estradiol, FSH, progesterone, total cholesterol and triglycerides. No variable differed significantly between the cream and placebo phases; both groups reported minor symptom improvement consistent with placebo response. This aligns with the underlying chemistry. Russell Marker's conversion of diosgenin to progesterone required a multi-step laboratory synthesis with reagents no mammal possesses, and no human enzymatic pathway performs it. Regulators have repeatedly found "natural wild yam" creams adulterated with synthetic progesterone, which explains anecdotal reports of real hormonal effects. Women seeking evidence-based options for vasomotor symptoms have better-supported choices, including hormone therapy where appropriate and several non-hormonal prescription agents.

    Caveats

    Treatments that do work for vasomotor symptoms include hormone therapy - the most effective option for those without contraindications - and non-hormonal prescription alternatives such as SSRIs and SNRIs, gabapentin, clonidine and the newer neurokinin-3 antagonists, plus cognitive behavioural therapy. Discussing these is more useful than trialling wild yam. Any postmenopausal bleeding needs urgent investigation. Safety: wild yam can cause nausea, vomiting and diarrhoea in larger amounts. Because some creams sold as wild yam contain undisclosed hormones, women with hormone-sensitive cancers, endometriosis or uterine fibroids should avoid these products entirely. Not recommended in pregnancy or breastfeeding - traditional use as an antispasmodic is not supported by safety data. It may theoretically interfere with oestrogen-containing medication.
