    Correct deficiency for bone and general health, but do not use vitamin D as SAD treatment. Light therapy is the intervention with real evidence.

    How It Works

    Vitamin D receptors are present in brain regions governing mood, and winter deficiency coincides with symptom onset, which produced a plausible but ultimately confounded hypothesis: reduced sunlight causes both low vitamin D and low light exposure independently.

    Evidence is insufficient. Doses of 1000-2000 IU/day and single high loading doses have been examined for seasonal affective disorder, and the trials are small, inconsistent and largely negative; the large VITAL-DEP trial of 2000 IU/day found no prevention of depression. The seasonal light hypothesis is compelling, but bright light therapy (10,000 lux for 30 minutes each morning) is the treatment with strong randomised evidence, alongside CBT and SSRIs.

    What the studies say

    VITAL-DEP randomised over 18,000 adults to 2000 IU daily and found no reduction in depression incidence or change in mood scores over a median five years. Earlier small trials in SAD produced mixed results with inconsistent methods. Meta-analyses of vitamin D for depressive symptoms find at most a small effect confined to deficient populations with clinical depression. Against light therapy, which has effect sizes comparable to antidepressants in SAD specifically, vitamin D is not a competitive option.

    Treatment with vitamin D3 compared with placebo did not result in a statistically significant difference in the incidence and recurrence of depression or clinically relevant depressive symptoms or for change in mood scores. These findings do not support the use of vitamin D3 in adults to prevent depression.

    There were no significant between-group differences in the primary (SIGH-SAD) and secondary (WHO-5) as well as the exploratory outcome measures. Thus, the study failed to demonstrate an effect of vitamin D on SAD symptoms, but our findings may be limited by confounders.

    Substituting vitamin D for light therapy delays effective treatment through the season. Avoid sustained doses above 4000 IU daily without monitoring.

    Less likely to help if

    The VITAL-DEP result suggests essentially everyone with seasonal mood symptoms and adequate vitamin D.
