    How It Works

    Riboflavin is the precursor to FAD/FMN, essential cofactors in mitochondrial electron transport — correcting the mitochondrial energy deficit implicated in migraine brain metabolism.

The landmark trial by Schoenen and colleagues randomised 55 patients to riboflavin 400 mg/day or placebo for 3 months and found a 59% responder rate versus 15% for placebo, with attack frequency roughly halved. Subsequent trials and meta-analyses have largely supported it in adults, though paediatric trials have been negative. Riboflavin is a precursor of FAD, required for mitochondrial electron transport, and the rationale rests on evidence of impaired mitochondrial energy metabolism in migraine. It reduces attack frequency, not attack severity or duration.

    Safety

    Caveats

    Harmless bright yellow urine is expected; occasional nausea. Standard B-complexes contain far too little — a dedicated 400 mg product is needed.

    Less likely to help if

    Benefit builds slowly — those judging at 4 weeks will wrongly conclude non-response; effects appear from month 2.
