- Evidence is insufficient. Interest arose because glutathione depletion is well documented in HIV infection, and in vitro work showed NAC could inhibit NF-kB-driven HIV replication in cell culture. Human trials at 600-1,800 mg/day did show restoration of glutathione and improvements in some immune markers, but they did not demonstrate reductions in plasma HIV viral load. The same pattern holds for other viral infections - hepatitis, influenza, SARS-CoV-2 - where NAC has been examined for oxidative stress, mucolysis or inflammatory outcomes rather than viral replication, and where trials have not shown meaningful reductions in viral load. NAC modifies host redox state; it does not interfere with viral replication machinery the way antiviral drugs do.
