    Genuinely effective in the specific case of iron-deficient restless legs disrupting sleep. Useless for sleepiness from apnea, insufficient sleep or medication.

    How It Works

    Iron is a cofactor for tyrosine hydroxylase in dopamine synthesis, and brain iron deficiency in the substantia nigra underlies restless legs syndrome even when peripheral stores appear adequate. Periodic limb movements and the urge to move fragment sleep architecture without full awakening, producing unrefreshing sleep and daytime sleepiness. Iron deficiency anaemia contributes separately through reduced oxygen delivery, though that produces fatigue more than true sleepiness.

    Evidence

    What the studies say

    The evidence for iron in restless legs is reasonably strong, with randomised trials of both oral and intravenous iron showing symptom improvement, and guidelines recommending repletion when ferritin falls below 75 ng/mL — well above the threshold used for general anaemia, because brain iron requirements differ from systemic ones. Reduced periodic limb movements and improved sleep continuity follow, and daytime sleepiness improves in parallel where restless legs was the driver. Outside this scenario the evidence weakens sharply. Iron does not improve sleepiness in people with normal ferritin, and treating fatigue with iron on the assumption of deficiency is a common error that delays diagnosis of sleep apnea, the far more prevalent cause. Alternate-day dosing improves absorption over daily dosing by reducing hepcidin induction.

    Caveats

    Investigate the cause of iron deficiency rather than simply replacing, particularly in men and postmenopausal women where gastrointestinal bleeding must be excluded. Oral iron causes constipation and nausea; alternate-day dosing improves both tolerance and absorption. Avoid iron where ferritin is normal.

    Less likely to help if

    Anyone with normal ferritin, and everyone whose sleepiness stems from sleep apnea, insufficient sleep opportunity or sedating medication.
