    Gotu Kola for Leg Swelling

    Gotu kola contains triterpenes that support venous and connective tissue integrity, and several trials show it reduces ankle swelling and symptoms of chronic venous insufficiency. A reasonable adjunct for bilateral venous swelling, but compression remains the mainstay.

    Overview

    Gotu kola has the strongest evidence of any herb for venous leg swelling. In a placebo-controlled dose-ranging trial in venous hypertension, the total triterpenic fraction of Centella asiatica reduced ankle oedema and capillary filtration rate, with a clear dose-response.
    Two things make that result unusually credible. Capillary filtration rate is an objective instrument measurement rather than a symptom score, and the higher dose outperformed the lower one, which is the pattern you expect from a genuine drug effect rather than noise. The pairing is specific to swelling caused by venous insufficiency. Leg oedema from heart failure, kidney disease, low albumin or a medication side effect is a different problem, and gotu kola is not an answer to any of them.

    How It Works

    Venous swelling happens because raised pressure in the leg veins forces fluid out of leaky capillaries faster than the lymphatics can clear it. The triterpenes in gotu kola, asiaticoside and madecassoside, act on the vessel wall itself.
    They stimulate collagen and glycosaminoglycan synthesis in the connective tissue surrounding capillaries and veins, improving structural integrity and reducing permeability. The measured drop in capillary filtration rate is the direct expression of that effect. Because the mechanism is connective tissue remodelling, it is slow. Trials run for four to eight weeks, and nothing meaningful should be expected in the first fortnight.

    Sudden one-sided swelling is a red flag

    New swelling in one leg, especially with pain, warmth or redness, can indicate a deep vein thrombosis. Seek same-day medical care rather than starting a supplement.

    Evidence

    The linked randomised, placebo-controlled dose-ranging trial is the anchor. In 87 participants with venous hypertension, both doses of the total triterpenic fraction reduced ankle oedema and capillary filtration rate, with the higher dose performing better.

    Safety

    Oral gotu kola at standardised doses is generally well tolerated over the weeks studied. Mild nausea, headache and drowsiness are the usual complaints, and contact dermatitis occurs with topical use.
