    Anthraquinones including emodin do relieve constipation, but tolerance, cramping and long-term mucosal concerns make them a short-term tool at best.

    How It Works

    Anthraquinones are activated by colonic bacteria to their aglycone forms, which irritate the colonic mucosa, stimulate propulsive peristalsis and reduce sodium and water reabsorption, increasing stool water content.

    Evidence

    What the studies say

    Clinical evidence for anthraquinone laxatives as a class is solid for short-term relief: senna and cascara consistently beat placebo for stool frequency, and guidelines list stimulant laxatives as second-line after osmotics and fibre. Isolated emodin has not been trialled as a laxative in humans; it is studied as a constituent. Against the benefit, chronic use is associated with melanosis coli (benign but a marker of ongoing use), electrolyte loss, and dependence-pattern use. EFSA and several national regulators have restricted hydroxyanthracene derivatives over genotoxicity concerns from emodin and aloe-emodin. For most people, magnesium, fibre and hydration are better long-term levers.

    Safety

    Caveats

    Not for daily long-term use. Cramping and urgency are common. Avoid in pregnancy, breastfeeding, IBD, bowel obstruction and liver disease. Interacts with digoxin and diuretics through potassium loss.

    Less likely to help if

    People with constipation from low fibre, low fluid intake or inactivity, and anyone with opioid-induced or obstructive constipation.
