    How It Works

    The pain of a herniated disc is substantially inflammatory: extruded nucleus material triggers cytokine release (TNF-alpha, IL-1, IL-6) around the nerve root. Curcumin inhibits NF-kB and downstream inflammatory signalling, which could in principle reduce this perineural inflammation and the pain it generates.

    Evidence

    What the studies say

    Trials of curcumin in chronic low back pain and osteoarthritis show small-to-modest pain reductions versus placebo, and preclinical studies show reduced radicular inflammation. There are no quality RCTs in acute disc herniation specifically. Reasonable as a short-term adjunct to the interventions that actually change outcomes — staying active, physiotherapy and adequate analgesia — but not a treatment for the disc itself, and it must never delay assessment of red-flag symptoms like progressive weakness.

    Caveats

    Mixed and indirect evidence — extrapolated from general inflammatory pain, not from disc-specific trials. Natural recovery makes personal improvement hard to attribute. Bleeding risk with anticoagulants and NSAIDs; stop 2 weeks before any spinal procedure or surgery. Rare hepatotoxicity. Avoid therapeutic doses in pregnancy.

    Less likely to help if

    Nerve-root compression with true neuropathic pain or motor loss — these need imaging and specialist care.
