    What the studies say

    Meta-analyses report modest HbA1c and fasting-glucose reductions in type 2 diabetes, but results are highly heterogeneous and larger, better-controlled trials trend null. Chromium deficiency in the general population is rare.

    Caveats

    Results across meta-analyses are highly heterogeneous and shrink in larger, better-controlled trials, so treat any benefit as modest and uncertain. Additive hypoglycaemia with insulin, sulfonylureas and metformin - monitor glucose and involve your prescriber before starting. Keep to 1000 mcg/day or below; case reports at high chronic doses describe kidney failure, hepatotoxicity and rhabdomyolysis. Avoid in kidney or liver disease. Interactions with levothyroxine and NSAIDs; antacids and calcium carbonate reduce absorption. Avoid high-dose chromium in pregnancy and breastfeeding. Never substitute it for prescribed diabetes therapy.

    People with normal glucose tolerance and adequate chromium status, and anyone with type 1 diabetes, where the mechanism does not apply.
