    How It Works

    As an antioxidant that regenerates other antioxidants and improves nerve blood flow, ALA reduces oxidative stress in damaged nerve tissue. It has demonstrated nerve-conduction and symptom benefits in neuropathy trials.

    Evidence

    What the studies say

    ALA's strongest evidence is in diabetic peripheral neuropathy, where multiple RCTs (the SYDNEY and NATHAN trials) showed symptom and nerve-function improvements at 600 mg daily. Evidence in radicular/sciatic pain is limited to small preliminary studies — mechanistically reasonable, clinically unproven. Sciatica's natural history (most cases improve in 6-12 weeks) and exercise-based care carry the real evidence, and ALA should not delay proven management or assessment of red flags.

    Thioctic acid 600 mg/day appears to be at least as effective as ALC in the treatment of sciatic pain caused by a herniated disc and may be associated with an improvement in symptom scores and reduced need for analgesia. However, because of the limited number of patients evaluated and the lack of a placebo control in this trial, further studies are warranted in order to provide more definitive results.

    Generally well tolerated; possible nausea or heartburn. Can lower blood sugar — caution alongside diabetes medication.

    Less likely to help if

    People with significant disc herniation or spinal stenosis causing structural compression, where the mechanism is mechanical rather than metabolic.
