Methodology
Double-blind randomised trial in 480 Chinese adults with mild or moderate hypertension: enalapril 10 mg alone, or with folic acid 0.4 mg or 0.8 mg daily for 8 weeks; homocysteine response analysed by MTHFR C677T and MTR A2756G genotype.
Key Findings
Homocysteine fell across genotypes and doses. With 0.8 mg folic acid, TT carriers had a larger fall than CC carriers at week 8 (22.0% vs 10.8%, P = 0.005); with 0.4 mg there was no difference (11.2% vs 9.9%).
Conclusions
MTHFR C677T genotype modified the homocysteine response to folic acid dose; TT carriers responded more to the higher of two physiological doses.
Limitations
Short (8 weeks); biochemical endpoint only; single setting with low background folate; genotype subgroups were small.
