Methodology
Double-blind, placebo-controlled crossover trial of 30-day oral diamine oxidase (1 capsule twice daily before meals) in 22 CSU patients incompletely controlled on antihistamines.
Key Findings
Diamine oxidase reduced 7-day Urticaria Activity Score by 3.8 ± 1.2 points versus placebo only in patients with low baseline serum DAO (p = 0.041); improvement was inversely correlated with baseline DAO and antihistamine use fell slightly.
Conclusions
DAO supplementation may relieve symptoms in the subgroup of CSU patients with low serum DAO levels.
Limitations
Very small sample (20 completers), benefit limited to a subgroup, short 30-day exposure, crossover design with possible carryover.
