Key Findings
Black cohosh did not alter serum estradiol (MD=1.24, 95% CI -4.58 to 7.08) or luteinizing hormone levels, but reduced hot flashes/sweating, insomnia, fatigue and vaginal dryness and lowered Kupperman menopausal index scores.
Conclusions
Limited evidence indicates black cohosh improves low-estrogen perimenopausal symptoms without changing hormone levels.
Limitations
Chinese-language trials of variable quality, small pooled sample, symptom-based endpoints, surgical/GnRHa population may not generalize.
