    A systematic review and meta-analysis of double-blind placebo-controlled gluten rechallenge studies to quantify the proportion of patients with presumed NCGS who demonstrate a genuine gluten-specific symptomatic response.

    Pooled data from rechallenge trials measuring symptom scores after gluten versus placebo exposure in patients self-reporting gluten sensitivity.

    Key Findings

    A relatively small proportion of self-reported NCGS patients showed a reproducible, gluten-specific symptomatic response distinguishable from placebo.

    Conclusions

    Findings support that self-reported NCGS is frequently not attributable specifically to gluten, reinforcing the importance of blinded challenge for diagnosis.

    Limitations

    Limited number of eligible RCTs, small individual trial sizes, and heterogeneity in gluten dose and challenge duration.
