Impact of Vitamin D Supplementation on Cognition in Adults With Mild to Moderate Vitamin D Deficiency: Outcomes From the VitaMIND Randomized Controlled Trial

Methodology
Two-arm parallel 24-month randomised placebo-controlled trial, delivered remotely, in 620 adults aged 50 or over with mild to moderate vitamin D deficiency and early cognitive impairment; primary outcome executive function via Trail Making Test Part B, with secondary measures of cognition, function and well-being plus subgroup and per-protocol analyses.
Key Findings
The VitaMIND trial randomised 620 adults aged 50 and over with mild to moderate vitamin D deficiency and early cognitive impairment to daily vitamin D or placebo for 24 months, run entirely remotely from participants' homes. Vitamin D produced no significant benefit on the primary outcome of executive function measured by Trail Making Test Part B (between-group difference 5,770, 95% CI -2,189 to 13,730). No benefit appeared on any secondary measure of cognition, function or well-being. Pre-defined subgroup and per-protocol analyses were also null. This is a clear negative result in a well-powered two-year trial.
Conclusions
Vitamin D supplementation produced no measurable improvement in cognitive outcomes in older adults with mild to moderate vitamin D deficiency.
Limitations
Participants had only mild to moderate deficiency, so the findings do not address severe deficiency, and the remote home-based design relies on self-administered cognitive testing and unverified adherence.
