    Effects of glucosamine-chondroitin combination on synovial fluid IL-1beta, IL-6, TNF-alpha and PGE2 levels in internal derangements of temporomandibular joint

    Methodology

    Randomised controlled trial of oral glucosamine-chondroitin versus control for 8 weeks in TMJ internal derangement, with synovial fluid cytokine and PGE2 assays plus pain and mouth-opening measures.

    Key Findings

    In 31 patients with internal derangement of the temporomandibular joint, oral glucosamine-chondroitin for 8 weeks reduced synovial fluid concentrations of IL-1beta, IL-6, TNF-alpha and PGE2 and improved pain and maximum mouth opening compared with the control group.

    Conclusions

    Glucosamine-chondroitin reduced inflammatory mediators in TMJ synovial fluid alongside symptom improvement, an interesting mechanistic signal from a very small trial.

    Limitations

    Tiny sample from a single centre, short 8-week duration, biomarker endpoints of uncertain clinical importance, invasive synovial sampling limiting participant numbers, and no long-term functional follow-up.

    These results might suggest that glucosamine-chondroitin combination significantly increases the MMO and decreases the synovial fluid IL1 and IL6 levels in internal derangements of TMJ compared to tramadol.
