Galacto-Oligosaccharide RP-G28 Improves Multiple Clinical Outcomes in Lactose-Intolerant Patients

Methodology
Multicentre randomised, double-blind, placebo-controlled trial of escalating-dose RP-G28 for 30 days followed by a 30-day real-world dairy reintroduction period, with patient-reported symptom and tolerance endpoints.
Key Findings
In 377 lactose-intolerant patients, 30 days of the galacto-oligosaccharide RP-G28 significantly improved abdominal pain and overall symptom scores versus placebo, and after 30 days of dairy reintroduction post-treatment more RP-G28 patients considered themselves lactose tolerant, consistent with colonic microbiome adaptation.
Conclusions
A short course of a targeted prebiotic galacto-oligosaccharide can durably reduce lactose intolerance symptoms by adapting the gut microbiome, offering an alternative to lifelong dairy avoidance or lactase pills.
Limitations
30-day treatment with limited long-term follow-up, self-reported symptom endpoints, industry sponsorship of the product under development, and results may not generalise to those with other functional GI disorders.
