    Methodology

    Updated systematic review and random-effects meta-analysis of observational cohort and cross-sectional studies reporting serum magnesium or hypomagnesaemia in PPI users versus non-users.

    Key Findings

    Meta-analysis of observational studies (129,347 participants) found proton pump inhibitor use was associated with a significantly increased risk of hypomagnesaemia (pooled OR approximately 1.44), with risk higher in patients concurrently taking diuretics and in longer-duration users.

    Conclusions

    The association between the exposure to PPI and the incidence of hypomagnesemia remained unclear.

    Limitations

    Entirely observational data with confounding by indication and comorbidity; heterogeneous hypomagnesaemia thresholds across studies; magnesium measured as serum level, a poor proxy for total body stores; no randomised evidence available.
