Methodology
Meta-analysis of 39 randomized trials of garlic preparations on total cholesterol, LDL, HDL and triglycerides.

Garlic reduced total cholesterol by about 0.28 mmol/L and LDL by 0.24 mmol/L in participants with elevated baseline cholesterol treated for over 2 months; no effect on HDL or triglycerides.
Conclusions
Garlic produces modest cholesterol reductions with longer-duration use in hypercholesterolemic people.
Limitations
Publication bias likely; many trials of low methodological quality.
