Systematic review and network meta-analysis of randomized controlled trials of pharmacological treatments for insomnia disorder in adults, ranking efficacy (acute and long-term) and acceptability with CINeMA confidence assessment.
Key Findings
In a network meta-analysis of 154 trials, benzodiazepines, eszopiclone, zolpidem and lemborexant were most effective acutely, while melatonin, ramelteon and non-licensed drugs showed no material benefit; tolerability and long-term data were weakest for the most effective agents.
Conclusions
Melatonin, ramelteon, and non-licensed drugs did not show overall material benefits.
Limitations
Sparse long-term data, heterogeneous outcome scales, few head-to-head comparisons, and many trials at moderate to high risk of bias.
