    Vitex agnus castus for premenstrual syndrome: a systematic review

    Abstract

    Systematic review of 17 RCTs showing vitex significantly superior to placebo for PMS symptom relief across multiple studies, with consistent benefits for breast tenderness, mood, and overall symptoms.

    Methodology

    Systematic review of randomised controlled trials of Vitex agnus castus preparations versus placebo or active comparators for premenstrual syndrome and premenstrual dysphoric disorder, with narrative synthesis of efficacy, tolerability and study quality.

    Key Findings

    Most included trials reported Vitex agnus castus to be superior to placebo for overall premenstrual symptom scores, including breast tenderness and mood symptoms, with generally mild adverse effects. Methodological quality was variable and several trials were funded by extract manufacturers.

    Conclusions

    Vitex agnus castus is a reasonable evidence-supported option for premenstrual symptoms, though effect sizes and the independence of the trial evidence are uncertain and higher-quality independent trials are needed.

    Limitations

    Small number of eligible trials, heterogeneous extracts and doses, variable outcome scales, industry funding in several studies, and risk of publication bias.

    Supplements Studied

    All eight studies were positive for VAC in the treatment of PMS or PMDD and VAC was overall well tolerated.
