    Key Findings

    In a crossover trial in military personnel with treatment-refractory PTSD, nabilone reduced nightmare severity on the CAPS recurring-dream item compared with placebo, with improvements also reported in global clinical impression and general wellbeing.

    Conclusions

    In this small sample NAB provided significant relief for military personnel with PTSD, indicating that it shows promise as a clinically-relevant treatment for patients with nightmares and a history of non-response to traditional therapies.

    Limitations

    Very small sample (n=10), short treatment periods, potential unblinding from psychoactive effects, and a highly selected non-responder population limiting generalisability.
