    Methodology

    Twelve-week randomized double-blind placebo-controlled add-on trial of B. bifidum Bf-688 (5 x 10^9 CFU/day) versus placebo in children with ADHD on stable methylphenidate, with CPT/CATA testing and faecal microbiome sequencing.

    Key Findings

    The Bf-688 group, but not placebo, showed significant improvements in CPT omission errors and Hit reaction time on both CPT and CATA, alongside an increased Firmicutes/Bacteroidetes ratio.

    Conclusions

    Add-on B. bifidum Bf-688 may enhance neuropsychological performance in medicated children with ADHD, possibly via gut microbiome changes.

    Limitations

    Small single-country sample; add-on design on top of stimulant medication; computerised attention tests rather than clinical rating scales as primary endpoints.
