    TL;DR

    Prostate size reduction means measurably shrinking gland volume, not just easing symptoms. Only 5-alpha-reductase inhibitors reliably do it — roughly 20-25% over six to twelve months. No supplement has demonstrated meaningful volume reduction in a good trial.

    Why It Matters

    Prostate volume predicts progression. Men with glands above 30-40 mL have a substantially higher risk of acute urinary retention and of eventually needing surgery, and volume is the main factor determining whether a 5-alpha-reductase inhibitor is worth adding to an alpha-blocker. Importantly, volume and symptoms correlate only loosely — some men with a 70 mL gland have mild symptoms while others with a 35 mL gland are severely affected, because the dynamic smooth-muscle component and detrusor changes matter as much as the mechanical one. That is why symptom relief and volume reduction are separate goals with different treatments.

    How to Measure

    Transrectal ultrasound is the standard measurement and calculates volume from three dimensions using the ellipsoid formula. Abdominal ultrasound is less accurate but non-invasive and adequate for tracking. MRI gives the most precise volumes and is increasingly used where cancer is also being evaluated. Because change is slow, repeat imaging is only meaningful at six to twelve month intervals. PSA serves as a rough surrogate — it correlates with volume and halves predictably on 5ARI therapy, so an unexpected rise on treatment warrants investigation.

    Optimization Protocol

    The only reliably effective interventions are pharmacological. Finasteride 5 mg daily inhibits type 2 5-alpha-reductase and reduces prostate volume by roughly 20-25% over six to twelve months; dutasteride 0.5 mg inhibits both isoforms, suppresses DHT more completely and produces a slightly larger reduction. Both take months, so they suit men with glands above 30-40 mL who need progression control rather than fast symptom relief, and both must be paired with the knowledge that they halve PSA. For faster symptom control while waiting, add an alpha-blocker — combination therapy outperforms either alone in men with larger glands. Beyond drugs, the levers are indirect: weight loss and improved insulin sensitivity are associated with slower prostate growth in cohort studies, as is regular physical activity, though neither has been shown to shrink an already-enlarged gland. Where volume is very large or medication fails, minimally invasive procedures such as Rezum water vapour therapy or prostatic artery embolisation reduce volume mechanically. Saw palmetto, despite its popularity, produced no measurable volume change in the STEP trial.
