Overview
Ankylosing spondylitis (axial spondyloarthritis) is a chronic inflammatory disease targeting the sacroiliac joints and spine. Its hallmark is inflammatory back pain: stiffness worst in the morning or after rest, improving with movement, in someone under 45. Untreated, chronic inflammation can fuse vertebrae, fixing the spine in a bent posture and restricting chest expansion. The HLA-B27 gene is present in most patients, though most carriers never develop disease. Diagnosis combines clinical features, inflammatory markers, and MRI of the sacroiliac joints. Modern treatment — sustained exercise programs, NSAIDs, and biologics (TNF and IL-17 inhibitors) — controls inflammation and largely prevents fusion when started early.

Root Causes
The root cause is genetic-immune dysregulation, not posture, injury, or diet. Flares, however, track with treatment interruption and smoking — smoking accelerates spinal fusion and worsens outcomes, making cessation a core intervention.
